Disability Income Protection Insurance |
| Eligibility | You are eligible to apply for coverage if you are a full-time active employee working 30 or more hours per week. |
| Premium | Your premium will remain the same until age 65. You will receive a 20% premium discount and have the convenience of payroll deduction by purchasing your coverage through your workplace. |
| Individually Owned | You can take the coverage with you if you change jobs (you would retain the 20% premium discount). |
| Medical Evaluation | Medical history and other factors will be taken into consideration when determining your eligibility for coverage. |
| Elimination Period | The elimination period is the number of days that must elapse in a Disability period before benefits become payable. These days need not be consecutive. You can choose to have your benefits begin after an elimination period of 60, 90, 180, or 365 days of disability. |
| Benefit Amount | Monthly disability benefit: Your choice in $100 increments to approximately 60% of your salary.
"Basic monthly earnings" means the insured's monthly rate of earnings from the employer in effect just prior to the application date. It does not include commissions, bonuses, overtime pay and other extra compensation. A Catastrophic Disability Benefit can be added to your income benefit to replace up to 100% of your prior income (not to exceed $8,000 per month). Call for details. |
| Benefit Integration | Your disability benefit will not be reduced by Social Security or other income replacement benefits you receive for the same disability. |
| Benefit Duration | You can choose how long you would like for disability benefits to be paid; 2 years, 3 years, 5 years, To age 65, or To age 67. |
| Definition of Disability | You would be considered disabled and eligible for disability benefits if, because of covered injury or sickness:

| Gainful Occupation | Gainful occupation means any occupation for which you are suited based on education, training, or experience, and can be expected to provide you with an income of at least 60% of your prior earnings; and are under the care of a physician. If you can return to any occupation but do not do so, a monthly income benefit of 50% is payable for up to the remainder of the benefit period. |
| Taxation | Under current tax laws your disability insurance benefit will not be taxed because you are paying for the coverage with post-tax dollars. |
| Instances When Benefits Would Not Be Paid | Benefits will not be paid for disabilities caused by, contributed to by, or resulting from: • intentionally self-inflicted injuries; • active participation in a riot; • war, declared or undeclared, or any act of war; • undisclosed pre-existing conditions (see definition below). Unum will not pay benefits for any period of disability during which you are incarcerated. |
| Pre-existing Condition | During the first two years from the Effective Date benefits will not be paid for a disability caused by a Pre-existing condition that was not diclosed, or that was misrepresented, in answer to a question on the application for this policy. Pre-existing condition means a sickness or physical condition for which within the two-year period prior to the Effective Date: 1) symptoms existed that would cause an ordinarily prudent person to seek advice or treatment from a Physician; or 2) advice or treatment was recommended by or received from a Physician. |
| Mental and Nervous Benefits | LTD benefits would be paid for 24 months per lifetime for disabilities caused by mental illness that meet the definition of disability. Mental and nervous benefits would continue beyond 24 months only if you are institutionalized or hospitalized as a result of the disability. |
| Waiver of Premium | You will not be required to pay premiums as long as you are receiving benefits. |
| Non-Cancellable Policy | This a non-cancellable policy. The Company cannot make changes to your policy or change your premium prior to age 65, nor can the policy be cancelled as long as you pay your premiums on a timely basis. |
